Anti-Wrinkle Injections vs Fillers: Which One Do You Actually Need?

Anti-wrinkle injections and dermal fillers get talked about as if they were two versions of the same thing. They aren’t. They’re different products, doing different jobs, in different layers of your face — and booking the wrong one is the single most common reason people leave an aesthetics appointment feeling that nothing much changed.

The good news is that the distinction is genuinely simple once someone explains it properly. Here’s the honest version.

The quick answer

Anti-wrinkle injections relax. Dermal fillers fill.

Anti-wrinkle injections use botulinum toxin to soften the muscle that creates a line every time you frown, squint or raise your brows. Take the movement down and the crease above it smooths out. The effect usually lasts around three to four months.

Dermal fillers are a hyaluronic-acid gel placed under the skin to replace volume or build structure — a flatter cheek, a soft jawline, a hollow under the eye, thin lips. Nothing is relaxed; something is added. Fillers typically last six to eighteen months depending on the product and the area.

So the shortcut is: if the thing bothering you only appears when you move your face, that’s usually anti-wrinkle territory. If it’s there when your face is completely still — a flattened cheek, a lost jawline, a deep fold, thin lips — that’s usually filler.

What anti-wrinkle injections actually do

A tiny amount of botulinum toxin is placed into a specific muscle. That muscle stops contracting as strongly, so the skin folded over it stops creasing, and the line softens. It is a muscle treatment that happens to improve skin — not a skin treatment.

That’s why it works so well on the upper face, where lines come almost entirely from movement: the vertical ‘11’s between the brows, horizontal forehead lines, and the fan of crow’s feet beside the eyes. It’s also why it does very little for a hollow cheek — there’s no overactive muscle to calm down.

A few things people are often surprised by. Results aren’t instant — you’ll notice a softening after a few days and the full effect at around two weeks. The treatment itself is quick, roughly 10 to 15 minutes. And it isn’t only cosmetic: the same treatment is used for jaw clenching and teeth grinding (masseter treatment) and for excessive underarm sweating. One honest limitation: if a line has been forming for years, it may have etched itself into the skin and still be faintly visible at rest even once the muscle is relaxed. Anti-wrinkle treatment will soften it considerably and stop it deepening, but a deeply set static line sometimes needs skin treatment or a little filler alongside it. Any practitioner promising a permanently blank forehead from injections alone is overselling.

What dermal fillers actually do

Fillers are a gel of hyaluronic acid — a sugar molecule your body already makes and uses to hold water in the skin. Placed under the skin, it does exactly what the name says: it takes up space. That space can be replacing volume you’ve lost, or building structure you never had.

Facial ageing isn’t only about lines. Fat pads shrink and shift, bone remodels, and the mid-face deflates — which is what drags the lower face down and deepens the folds either side of the mouth. The trade-off compared with anti-wrinkle injections: filler is more visible immediately, but there’s more short-term swelling and a real chance of small bruises for a few days. The reassurance is that hyaluronic-acid filler is dissolvable — an enzyme injection can remove it if something isn’t right or you change your mind. Nothing about it is permanent.

Can you have both?

Yes — and for most faces over about 40, a combination gives a better result than either treatment pushed to its limit on its own. They sit in different layers: toxin goes into muscle, filler sits above bone or under skin. They don’t compete for the same space.

The classic combination is relaxing the upper face while restoring support in the mid and lower face. That’s what most people are actually picturing when they say they want to look “less tired” — not a smoother forehead in isolation.

There’s also a good argument for notdoing everything at once. Doing one treatment, letting it settle for two weeks and then reassessing very often shows that the second thing you’d planned isn’t needed — or is needed somewhere different. Before you book anyone

Two things worth knowing whoever you choose, and they apply across the UK, not just here.

First, botulinum toxin is a prescription-only medicine. It has to be prescribed for you personally by a qualified prescriber following a consultation — it is not something that can legitimately be sold as an off-the-shelf treatment, and remote prescribing without seeing you is not acceptable practice. Dermal fillers are not classed as medicines in the same way, which is precisely why who is holding the needle matters so much.

Second, in England it has been a criminal offence since 1 October 2021 to administer botulinum toxin or cosmetic fillers by injection to anyone under 18for cosmetic purposes — including agreeing to book the appointment. Any clinic offering either treatment to a 17-year-old is breaking the law.
